Pediatric Fatty Liver Treatment in Silver Spring

This condition is common, and occurs most often in patients with overweight/obesity. It is estimated that 30%–50% of kids with obesity have MASLD.  It is heavily influenced by genetics, and we know that certain populations (for example, Mexican American males) are more commonly affected than their peers of other ethnicities. 

our pediatrician may suspect MASLD if your child has overweight/obesity and abnormal liver enzymes on blood tests. Sometimes the liver is also enlarged, which can be determined by a physical exam or ultrasound. 

In some patients, additional testing (often ordered by a pediatric gastroenterologist or hepatologist) is needed to confirm the diagnosis. This may include blood tests and imaging, and in some cases, a biopsy of the liver. 

The good news is that weight reduction is an effective treatment for MASLD. In many cases, the inflammation can be reduced (and in some cases totally reversed) with changes to the diet and increased physical activity. 

Currently there are no medications specifically approved for MASLD in pediatrics. There are prescription treatments that are used in adults with more advanced MASLD, and research on additional treatments for kids is ongoing. Wegovy (semaglutide) injection is approved by the FDA to aid in weight loss in patients as young as 12 years old. Other studies are ongoing to evaluate weight loss medications and MASLD-specific medications in children and adolescents 

Because the primary treatment in pediatric MASLD is weight loss, families may wish to seek out a specialized care team that focuses on weight reduction in children/adolescents.  An obesity medicine physician, or pediatric weight management program (if available in your area) would be a great place to start. 

The good news is that with increased awareness and screening efforts by pediatricians, MASLD is being diagnosed earlier in children. When identified and effectively treated early, this condition is fully reversible for many patients.

Once diagnosed, it is important to have regular follow-ups with your doctor to monitor progression. Without intervention, it is possible for the disease to progress (usually over many years) to cirrhosis of the liver.  In adults, fatty liver disease leading to end-stage liver is disease is now the #1 cause of liver transplants in the United States.
